oh I know what he means by safety mode...
the gun jammed on the loading of the second bullet, when the slide stuck open, that's what he thought was "safety mode"...
I use Fiocchi for my target ammo, and its really great stuff (nice bullet weight, hot powder load), but the truncated shape of the bullet sometimes doesn't load properly, so I have to really tap my magazines, to make sure all of the rounds are all the way to the back so it doesn't happen...

hey dahveed: that might be it about the second bullet didn't sit back enough, so it ejected when the first round's brass ejected out. btw, i use winchester white box and remington umc 9luger. what do you think about those ammo???? 2nd, i am planning to get a Glock 19C. any inputs???
scoobyboy: No worries, i was cleaning my gun and everything is just fine. i called up my friend and he told me if the gun has exploded, the gun might have been wasted.
