Korea, japan, france, sweden, etc are all small and dense countries. they can hook up their entire country relatively easily. we, on the other hand, only have super fast connections such as fiber optic in certain areas, and it isn't cheap.

Srsly though, Infrastructure. They're usually smaller, therefore easier to upgrade land lines, etc. The U.S. seems to be lagging in upgrading infrastructure in key areas within the last 15 years at least.

There are a number of reasons for this, population density is a big part of it though.
Realize that to string fiber optic cable, it's easy to justify the cost in a city or dense urban area, because the residences skipped, ie that won't be interested in your services, ex 40%, as a percentage is the same as in sparsely populated areas as in densely populated areas.
Stringing fiber to rural areas is very expensive, even pulling fiber to the suburbs can be difficult to justify...but in the city centers, a "take" rate of 60% just means money.
This is sort of what my company does, provide back haul and fiber services for the doughnut around the metro areas. There is money to be made there, but we pick and choose, because there is a lot of money to lose there too...ask some of the former dot com companies that tried to pull fiber everywhere...bankruptcy. Without a certain % utilization to be able to pay off your expenses, it's just not worth it.
Add into the mix the various ILEC's and system lata's brought in by congress...these also complicate the issue...in those other countries, I'll be they only have one phone company, greatly simplifying things....In the US we have LOTS of different carriers and we have lata's within each state that different rates apply to...joy.

None of this helps to carry phone or Internet over them cost effectively.

Easy. We were one of the first to install phones nation wide and data wires nationwide. Those wires are controlled mostly by one company and everyone else rents them. They are old. To replace them all would cost a FORTUNE. Americans are too stupid to understand how much they're being ripped off and how far behind everyone else they are. Our economy is ****ED up right now and all these big companies are taking it wherever they can. Like this AT&T/Verizon beef right now... it's all bull crap and they're all fighting over OLD technology.
I remember working with a south korean telecom company when I worked for AT&T in 03 or 04... they were comparing our stuff to theirs... back then they were doing everything we're fighting over now.... AND WAY MORE. People maintaining websites via their cell phones and things like that. BASIC internet connections installed in pretty much all housing where if I remember around 13x's faster than our FASTEST... and that was their basic stuff 6 years ago.
No one wants to pay to upgrade the lines and towers.
